By participating in this Spiritual Healing Group, I acknowledge and agree to the following:

I voluntarily receive spiritual healing offered by JewelTree Center for Spiritual Growth & Guidance, and I release the Center and its facilitators, Aden Sletta and Swami Sky, from any and all liability for any harm or perceived harm that may arise from this or future sessions.

I understand that Aden Sletta and Swami Sky are not licensed professionals in counseling, therapy, or medicine, and their work is spiritual in nature.

If I am under the care of a licensed medical or mental health professional, or currently taking medication, I agree to consult with them before making any changes to my treatment or care plan based on any perceived outcome of this healing session.

By registering for this event, I affirm that I have read, understood, and agree to the terms stated above.
